Forms that athletes must have completed and signed before trying out for, or practicing with, a district sports team are also attached below.
Sports Physicals:
If your child has already submitted a completed JPA-24 form signed off by a doctor to the MUSD within 12 months of trying out for an upcoming sport, please simply write the date of that physical at the top, write N/A, and sign. To continue in any sport a new clearance must be obtained before the 12 months run out.
For your information, Redwood Urgent Care is open daily from 9:00 a.m. - 7:00 p.m. (last admission at 6:30) and provides walk-in sports physicals for $50 at 2440 23rd Street in Eureka. Minors must be accompanied by an adult and payment is due at the time of service. Please be sure to take Pre-participation Form (aka JPA Form-24) with you to your physical (the 3rd and final page is the one schools must have current and on file.) This form is attached below and copies are available in the school office.
